Making a referral

Support coordinators, LACs, hospital discharge planners and allied health teams — we accept referrals across Melbourne and regional Victoria, including short-notice and complex requests.

Send through:

  • A copy of the current NDIS plan, or the relevant budget lines
  • A short participant summary, including risks and preferences
  • The supports and indicative hours sought
  • Location, and any worker gender or language preferences
  • Your contact details and the participant’s consent to share

Email info@sodacare.com.au with “Referral” in the subject line. We respond with capacity and indicative start dates within one business day.

Feedback & complaints

If something is not right, we want to hear about it. Compliments are welcome too — they tell us what to keep doing.

  • Tell any Soda Care worker or coordinator, or use the form on this page
  • We acknowledge complaints within 2 business days and aim to resolve them within 21 days
  • You can complain anonymously, or have a family member or advocate do it for you
  • Raising a concern will never affect the supports you receive

You can also go straight to the NDIS Quality and Safeguards Commission on 1800 035 544 or at ndiscommission.gov.au. In Victoria you can also contact the Victorian Disability Worker Commission on 1800 497 132.
